What patient information is needed?
- We are located in The Elizabeth ARCADE. This is a street-level office area adjacent to The Elizabeth condominium building. The garage is in-between the Arcade entrance and the horseshoe entrance in front of the building. As you descend into the garage, you will see a call box on your left with our business name and a code listed. When you enter the code (009), it calls our front desk and we will let you into the garage. Park in any spot within the BLUE parking area. Parking is FREE!
- If you have any trouble walking or navigating stairs, please let our admin assistant know and we will get you handicap accessibility into our office.
Do you take my insurance?
- We are Medicare preferred providers. Medicare has a yearly allowance for out-patient physical therapy services and covers 80% of your invoice. The additional 20% is either fully or partially covered by your secondary or supplemental insurance if you have one. Please call your secondary/supplemental insurance plan to find out exactly how much of the 20% remaining invoice they will cover.
- We are out-of-network with private insurance companies, BUT we file all claims for you and you only pay the co-insurance/deductible at each visit. We will collect the remainder of your invoice from your insurance company using your out-of-network benefits (except for BCBS and BCBS affiliates). Many times you pay the same as in-network providers for better service!
- BCBS patients (Carefirst, Federal and out of state BCBS) - we fill out your claims for you and you walk out of the office with your claim form and addressed envelope ready to go. Payment is made up-front and you will be reimbursed by your insurance company according to your benefits. We keep our services affordable and have several options. Please call our office to find out our up-front pricing.
